New SmartGut doctors & first time SmartGut doctor portal users

Email smartgutpilot@ubiome.com with your NPI number to request a provider account. Our Pilot Program team will send you an email confirmation. Sign into uBiome’s Provider Portal, enter the email address you used to request access, and click “I forgot my password.” You will receive a link in your email to establish a password.

Then, sign into uBiome’s Provider Portal using your new password. To view your patients’ reports click “View SmartGut Test(s).”

Prescribing SmartGut providers

If you are a physician who has already prescribed SmartGut to your patients, sign in to uBiome’s Provider Portal, a HIPAA-compliant online platform where you can easily access patient data and review the status of your patients’ test orders.

Your Provider Portal account is associated with the email address noted on your submitted test requisition form(s).

If you have not previously signed in to the Provider Portal but you sent in a TRF via fax or email, you can finish setting up your account:

Visit smartgut.ubiome.com.

Enter the email address you entered on the faxed or emailed TRF* and click “I forgot my password.” You will receive a link in your email to establish a password.

Then, sign into your Provider Portal using your new password.

To view your patients’ reports click “View SmartGut Test(s).”
